PLEASE NOTE THE FOLLOWING
All forms under the Kindergarten Registration Button are required to be completed. You may print the forms or pick up a packet at the W.R.Croman Primary School office, or the District Administration offices. Please call Tanya Mattocks to schedule an appointment to register your child, 570-297-2750 x2208.
Your student to enroll - must be 5 by August 31.
You will need to bring:
- Your student's birth certificate
- Your student's immunization records
- Two proofs of your residency in the Troy Area School District
- Custody papers - if applicable
Kindergarten Health Requirements
Since a child's overall health and well-being can directly affect his/her ability to learn, the Pennsylvania Department of Health have set requirements for immunizations and regular health screenings for school-aged children. Kindergarten students are required to have a physical and dental exam before or during their first year of school. Below you will find the links to the Physical and Dental forms, as well as the Pennsylvania Department of Health immunization requirements. You must bring a copy of your child's immunization record with you to registration. The nurse will then review them and inform you of what, if any further immunizations are needed for your child to start kindergarten.
Please print the Physical and Dental forms and have your child's provider complete them. Once completed please return to the nurse.

You will need to bring to your appointment:
- Your student's birth certificate
- Your student's immunization records
- Two proofs of your residency in the Troy Area School District
- The name and address of the school your student last attended.
- Custody paperwork if applicable.
- Note students in grades 9 - 12 - grade level will be determined by credits earned.(0-6 credits grade 9, 6-12 credits grade 10, 12-19 credits grade 11, 19-25 credits grade 12) Troy requires 25 credits to graduate.
- Please bring completed forms found under the Transfer Registration section.
