State Mandated Health Screenings 

Dental Examinations: K, 3rd and 7th grades 

Physical Examinations: K, 6th and 11th grades 

Exams will be accepted for up to one year before entering the appropriate grade to fulfill this requirement. Please forward to the appropriate nurse's office.  

Immunization Mandates:  

Before starting K- 4 DTP, 4 Polio, 2 MMR, 2 Varicella, 3 Hep B 

Before starting 7th grade- Tdap and MCV 

Before starting 12th grade- MCV 

Schools may not be open at this time, but our mandates still apply. Please help the school nurses by checking the mandates for your students and forward the appropriate information. Thank you for your assistance in this important matter. Stay safe during this interesting time in our lives!
