Physicals - Please forward a recent physical (dated 8/2020 to present) to the nurses office. You should have been notified personally if you still need to send this information to the Nurse.
Immunizations - Your student will need their second MCV BEFORE starting their senior year. Please have that completed now and have the doctors office forward the information to the nurse.Parents have been notified if the information is still needed.
