Dental Exams - Have your child's dentist forward a report from a recent visit (dated 8/2020 to present) to the school nurse office.
Immunizations - Your student is also in need of a tetanus booster and the first of their MCV immunizations to meet the state mandates.
Have their family physician forward this information if already completed or make an appointment to have them completed as soon as possible.
