Kindergarten registration is now open for children that will be 5 years old on or before August 31, 2026. Kindergarten Registration Checklist

Registration Packets are available to pick up at the W.R. Croman Primary School office, between 7:30 – 4:00 PM. Packets are also available here to print.

These forms are required for registration.

  1. Your student's birth certificate

  2. Your student's immunization records

  3. One proof of your residency in the Troy Area School District

  4. Custody papers - if applicable

Kindergarten Health Requirements

Since a child's overall health and well-being can directly affect his/her ability to learn, the Pennsylvania Department of Health have set requirements for immunizations and regular health screenings for school-aged children. Kindergarten students are required to have a physical and dental exam before or during their first year of school. Below you will find the links to the Physical and Dental forms, as well as the Pennsylvania Department of Health immunization requirements. You must bring a copy of your child's immunization record with you to register. 

Please print the Physical and Dental forms and have your child's provider complete them. Once completed please return to the nurse.
