Dental Exams - Have your child's dentist forward a report from a recent visit (dated 8/2020 to present) to the school nurse office if this has not already been completed.
Immunizations - Your student needed a tetanus booster and their first MCV immunization to start this school year. You have been notified if the nurse does not have this information. Have their family physician forward this information if already completed or make an appointment to have them completed as soon as possible.
